Job summary

The candidate will evaluate function, design and progress plans, supervise assistants, and document outcomes while collaborating with a multidisciplinary team to improve patient care. A BS in Physical Therapy is required.

Qualifications

• BS in Physical Therapy required.
• Current Maryland license as a Physical Therapist.
• Healthcare Provider (CPR Certification).
• Experience: None required; 2 yrs related experience preferred.

Responsibilities

• Evaluates patient functional status including review of medical history, diagnosis, and physician's prescription.
• Determines short and long-term goals with input from patient/caregiver; establishes plan of care based on goals and evaluation results.
• Implements therapeutic interventions, progresses treatment plan and teaches patients/caregivers activities.
• Documents evaluations, treatment plans, progress notes and discharge summaries; records data for statistics, billing, and quality improvement.
• Administers manual therapy interventions and supervises PTAs, Rehab Techs, and volunteers as required by regulations.
• Communicates with multidisciplinary team and patients/caregivers to discuss findings and progress.
• Participates in performance improvement activities and completes continuing education requirements.
