Responsibilities

• Demonstrates ability to perform patient transfers with assistance as needed.
• Maintains safe body mechanics to protect patients and staff.
• Develops and updates an individualized plan of care including goals and discharge plan.
• Educates patients and families using effective teaching methods.
• Performs examinations to determine PT diagnoses and outcomes.
• Documents care and progress according to policy.
• Communicates progress with referring providers.
