Job summary

Carle Health is seeking a licensed Physical Therapist to evaluate and treat patients of all ages, adjusting plans of care based on patient response and evidence. You will educate patients and families, document outcomes, and collaborate with the care team.

The role includes orthopedic and neurologic evaluations, daily therapy in EMR, and participating in a comprehensive stroke program with ongoing continuing education requirements. A sign-on bonus is available.

Qualifications

• Bachelor’s degree required in Physical Therapy.
• Licensed Physical Therapist (PT) with IDFPR.
• BLS within 30 days required.
• Continuing education on acute stroke care required.

Responsibilities

• Demonstrates ability to perform patient transfers with assistance as needed from bed, chair and floor.
• Treats patients with dignity and respect, remaining empathetic and engaged.
• Designs an appropriate plan of care and adjusts based on patient response and evidence.
• Educates patients, families, caregivers, students and other health care providers.
• Evaluates clinical findings to determine PT diagnoses and outcomes.
• Performs examination and selects appropriate assessment methods.
• Produces documentation per CTSCP67 policy.
• Screens patients to determine need for further therapy services.
• Monitors care delivered by support personnel and provides feedback.
• Completes annual education to comply with BMP requirements.
