Job summary

SKYLINEHEIGHTS is seeking a Staff Physical Therapist to evaluate residents, develop treatment plans, and work with the care team to improve mobility and independence. You will supervise PT assistants and educate staff while maintaining compliant documentation and communication with families and physicians.

The role requires a PT license in the state, with continuing education as required, and CPR certification if required.

Qualifications

• Bachelor's degree in Physical Therapy; Masters/Doctorate preferred.
• Proficient in Net Health and Point Click Care.
• Licensed Physical Therapist; maintain license; CPR if required.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and other staff in resident care.
• Communicate with physicians, nursing staff, families about progress and discharge planning.
• Complete timely, accurate documentation per policies and regulations.
• Provide in-services and staff training as needed.
