Job summary

Rivercrest is seeking a Staff Physical Therapist to evaluate residents, design personalized treatment plans, and supervise PT aides. You will collaborate with physicians, families, and the health team to achieve mobility goals and ensure safe, effective care.

The role requires a Physical Therapy license, a Bachelor's (Masters/Doctorate preferred), and proficiency with Net Health and PCC systems. CPR may be required. Strong communication and documentation skills are essential.

Qualifications

• Education and/or Experience: Bachelor's degree in Physical Therapy; Masters/Doctorate preferred.
• Proficient in computer skills for patient data entry and records (Net Health, PCC).
• Licensed as a Physical Therapist in the state; CPR certification may be required.

Responsibilities

• Evaluate residents to determine PT needs and rehab potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and staff in resident care activities.
• Maintain timely, accurate documentation per policies and payer requirements.
