Job summary

REHAB STRATEGIES in Indianapolis, IN is seeking a dedicated Physical Therapist to assess and implement care plans that improve mobility and function across inpatient units, outpatient clinics, home health, and community programs.

You will evaluate patients, apply manual therapy and exercises, coordinate discharge planning with a multidisciplinary team, educate patients and families, and document progress in EMR systems to guide ongoing care.

Qualifications

• Bachelor’s or Master’s degree in Physiotherapy or equivalent.
• Valid Physical Therapist licensure.
• Experience in inpatient, outpatient, home health, or community care.
• Experience with EMR documentation and reporting.

Responsibilities

• Assess patients and develop tailored treatment plans.
• Apply manual therapy, trigger point therapy, exercises, and modalities.
• Coordinate discharge planning with multidisciplinary teams.
• Educate patients and families on injury prevention and home exercise programs.
• Document patient progress in EMR and adjust treatment plans as needed.
• Help in acute care scenarios to promote rapid recovery and mobility.
