Woodland Health and Rehabilitation in Mount Pleasant, IA is seeking a dedicated Physical Therapist to evaluate, develop, and implement personalized treatment plans to improve mobility, reduce pain, and enhance quality of life. You will work across outpatient clinics, inpatient settings, and home health visits as part of a multidisciplinary team.

This role emphasizes collaboration, detailed documentation in EHR, patient and family education, and continuity of care across environments.

Qualifications

• Licensed Physical Therapist in the appropriate jurisdiction.
• Experience in outpatient, hospital, or inpatient settings.
• Knowledge of anatomy, physiology, orthopedics, neurology, geriatrics.
• Familiarity with EHR/EMR systems for patient data management.
• Experience developing treatment plans for varied conditions, including geriatrics.
• Ability to communicate effectively with patients of all ages.

Responsabilités

• Conduct comprehensive evaluations and develop tailored treatment plans.
• Implement manual therapy, trigger point therapy, and exercise programs.
• Monitor patient progress and adjust care plans as needed.
• Collaborate with physicians, nurses, OT, and other providers to coordinate care.
• Maintain accurate clinical documentation in EHR systems.
• Educate patients and families on exercises and home care strategies.
• Participate in outpatient, inpatient, and home health visits.
