Job summary

St Luke's Health - Patients Medical Center is seeking a licensed Physical Therapist to provide comprehensive physical therapy services to patients with impairments or functional limitations from injury or disease. You will develop and update plans of care with measurable goals and a discharge plan, tailored to the patient’s age and lifestyle.

You will work with an interdisciplinary team, meet productivity standards, document thoroughly, and participate in patient education.

Qualifications

• Degree in Physical Therapy
• Graduates eligible for licensure will be considered
• State Licensure in Physical Therapy (PT)
• CPR/BLS certification

Responsibilities

• Develop and update appropriate plan of care in collaboration with the patient/family and related to the persons age and lifestyle with measurable long and short term goals and a defined D/C plan
• Perform age appropriate competency skills in area of practice and with respect to individual patient problems.
• Discharge patient when goals have been met or medical necessity is no longer warrants intervention.
• Establish provisions and/or make recommendations
• Consistently meet patient care productivity standard as established for job classification.
• Works collaboratively with all members of the interdisciplinary team to assure patients needs are met in a timely manner.

Education

Degree in Physical Therapy

Graduates eligible for licensure will be considered

State Licensure in Physical Therapy (PT)

CPR/BLS certification
