Job summary

St. Luke's University Health Network is seeking a Physical Therapist to evaluate, diagnose, and manage patients with impairments to maximize functional abilities. You will design individualized treatment plans and educate patients and families to promote optimal health and wellness.

The role involves developing goals, using evidence-based therapies, and collaborating with a multidisciplinary team to ensure high-quality care across outpatient settings.

Qualifications

• Doctoral Degree, Masters or Bachelor’s degree from an accredited physical therapy program
• Minimum of one year of experience as a physical therapist in an outpatient setting
• CPR certification required
• Ability to educate patients and family members about prognosis and care options

Responsibilities

• Performs comprehensive initial evaluations to identify functional deficits and develops a treatment plan
• Provide skilled interventions tailored to the patient’s needs using appropriate techniques and equipment
• Collaborates with patients to create short and long-term goals
• Communicates effectively with patients, families, and other healthcare providers
• Establishes and updates diagnosis, prognosis, and plan of care based on evidence and clinical reasoning
• Monitors and documents patient progress and outcomes
• Supervises PTAs and students as assigned
• Maintains confidentiality and follows safety and regulatory standards
• Attends meetings and contributes to QI data collection and departmental activities
• Cleans and orders supplies for daily operations
• Attends multi-disciplinary meetings as applicable
