Job summary

Laurel Park Rehab and Healthcare Center in Hertford, NC is seeking a part-time Physical Therapist to join our growing team.

You will perform comprehensive PT evaluations and develop treatment plans based on MD orders. Responsibilities include documenting progress, adhering to guidelines, and coordinating with the interdisciplinary team to optimize patient outcomes. Qualifications

• Must have degree in Physical Therapy recognized by the APTA.
• Must hold current license/registration in the state of North Carolina.

Responsibilities

• Provide a comprehensive physical therapy evaluation based on MD orders.
• Document findings on standardized evaluation forms and guidelines.
• Develop a comprehensive treatment plan with goals, frequency, duration, and modalities.
• Obtain MD orders for evaluations, treatments, re-certifications, and discharges.
• Deliver comprehensive treatment to patients using modalities and adjust as needed.
• Document daily participation and weekly progress.
• Prepare discharge summaries and in-service documentation as required.
• Attend facility meetings and participate in interdisciplinary team activities.
• Supervise junior staff or students as needed and co-sign notes.

Education

B.S., Doctorate, MA or MS in Physical Therapy recognized by the APTA

Licensed/registered in the state (as applicable)
