Assisted Home Health & Hospice is seeking a Physical Therapist (PT) for Home Health. The role involves comprehensive patient assessments, development of care plans, and delivery of therapeutic interventions in patients' homes.

Candidate must be a licensed PT in Arizona with at least one year of experience, CPR/health clearance, and be prepared to engage with physicians and caregivers to optimize outcomes.

Qualifications

• Education and qualifications commensurate with § 484.115(h).
• Licensed as a physical therapist pursuant to ARS Title 32, Chapter 19 by the Arizona State Board of Physical Therapy.
• Core competency per skills checklist.
• At least one year of experience in physical therapy.
• Fingerprint card, DMV and criminal background check, as appropriate.
• Current CPR, TB and health clearance.
• Complete 6 hours of in-service training per year.

Responsabilités

• Complete initial and ongoing interdisciplinary assessment of the patient with consideration of level of functioning.
• Perform manual and electrical muscle exams, gait analysis, ROM and sensory testing.
• Develop and evaluate the plan of care with patient, family and caregiver in coordination with the physician and care team.
• Provide therapies ordered by the physician as part of the plan of care.
• Educate and counsel patient and family on therapy and equipment use as needed.
