Job summary

Green Hills Center for Rehabilitation and Healing is seeking a Certified Occupational Therapy Assistant (COTA) to implement treatment plans under the supervision of an Occupational Therapist. The role emphasizes precise documentation, timely progress notes, and collaboration across disciplines to deliver patient-centered care.

Experience in long-term or geriatric care is preferred, with a focus on professionalism, punctuality, and reliable performance within a rehabilitative setting.

Qualifications

• Must have a valid state license and registration.
• Provide comprehensive treatment under supervision of Occupational Therapist
• Experience in long-term care and/or geriatric care preferred
• Well versed with documentation writing
• Document patient's progress on weekly progress note in a timely fashion
• Collaborates with all disciplines to provide best patient centered treatment and care
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times
• Perform all other duties assigned by Director of Rehabilitation

Responsibilities

• Carry out best practice treatments under OT supervision.
• Provide feedback to the supervising occupational therapist via documentation or other required methods.
• Attend to patient progress documentation in a timely fashion.
• Collaborate with all disciplines to provide patient-centered care.
• Demonstrate professionalism, punctuality and reliability.
• Perform duties as assigned by the Director of Rehabilitation.
