Insight Global is seeking a Community Based Care Coordinator to manage care for individuals eligible for Medicare and Medicaid. You will assess medical, behavioral, and social needs in homes or community settings and develop personalized care plans.

You will coordinate with physicians, social services, and community organizations, leading ICTs to ensure smooth transitions of care and follow-ups after hospitalizations.

Competenze

• Must have experience in a healthcare-related profession (e.g., Nursing, Social Work, Counseling, Case Management).
• Experience coordinating care for dual-eligible Medicare/Medicaid populations is a plus.
• Knowledge of Medicare/Medicaid benefits and managed care programs.
• Experience conducting assessments in members' homes or community settings.
