CareSource is seeking a Community Based Care Coordinator, Duals Integrated Care to manage care for dual-eligible beneficiaries, coordinating Medicare and Medicaid services with community resources to improve outcomes.

The role requires nursing or healthcare licensure, strong care coordination, and ongoing collaboration with providers, ICTs, and families, with travel throughout Michigan to support members and ensure smooth transitions.

Competenze

• Nursing or healthcare background with care coordination experience.
• Familiarity with dual-eligible populations.
• Knowledge of Medicare/Medicaid programs and community resources.

• Coordinate care across providers and community resources.
• Lead interdisciplinary care team to develop care plans.
