Job summary

You will connect members with resources, coordinate transitions of care, and collaborate with interdisciplinary teams to prevent readmissions while maintaining thorough documentation and advocacy for patient-centered care.

Qualifications

• Must have experience in a healthcare-related profession (e.g., Nursing, Social Work, Counseling, Case Management, Care Coordination).
• Experience coordinating care and transitions of care for dual-eligible populations is highly relevant.
• Knowledge of Medicare/Medicaid programs and community resources is preferred.

Responsibilities

• Coordinate care for Medicare/Medicaid eligible individuals.
• Assess medical, behavioral, functional, and social needs in homes or community sites.
• Develop and update individualized care plans; connect to community resources.
