Insight Global seeks a Community Based Care Coordinator to arrange and coordinate care for Medicare/Medicaid members in home settings. You will assess needs, develop care plans, and connect members with community resources.

You will work with healthcare providers, ICTs, and community organizations to ensure smooth transitions of care and ongoing support. Strong licensure in Ohio is required.

Competenze

• Must hold a current, unrestricted clinical license in Ohio.
• Licenses include RN, ASN, Social Worker, Clinical Counselor, or relevant Master’s level licensure.
• Experience with Medicare/Medicaid populations preferred.

• Coordinate care across providers and community organizations.
• Assess medical, behavioral, functional, and social needs in members’ homes.
• Develop, implement, and update individualized care plans.
• Monitor progress and follow up after hospitalizations or health events to prevent readmissions.
• Advocate for members and ensure accurate documentation of care and outcomes.
