Job summary

Insight Global seeks a Community Based Care Coordinator to coordinate care for dual-eligible Medicare/Medicaid members in the Dayton, OH area. You will perform home assessments, develop care plans, connect members to resources, and collaborate with doctors, hospitals, and community partners.

The role requires licensure in Ohio (RN, LBSW/MSW, LCPC or equivalent), strong care coordination skills, and experience with chronic conditions, transitions of care, and patient advocacy.

Qualifications

• Must hold a current, unrestricted clinical license in Ohio (RN, Social Worker, or Clinical Counselor).
• Licenses and/or degrees must be active and in good standing.
• Experience coordinating care for dual-eligible Medicare/Medicaid populations is preferred.

Responsibilities

• Conduct comprehensive member assessments to identify medical, behavioral health, functional, and social needs.
• Develop, implement, and update individualized care plans.
• Meet with members in their homes and in community settings to coordinate care.
• Collaborate with physicians, hospitals, and community organizations to ensure seamless transitions of care.
