Job summary

Insight Global is seeking a Community Based Care Coordinator to coordinate care for Medicare/Medicaid beneficiaries living at home in Ohio. You will assess medical, behavioral, and social needs, develop individualized care plans, and connect members with community resources while working across healthcare providers and organizations.

You will lead ICTs, coordinate transitions, educate members on benefits, and advocate for patient participation in care decisions.

Qualifications

• Current, unrestricted clinical license in Ohio (RN or MSW or Clinical Counselor).
• Must have healthcare-related licensure in Ohio.
• Experience coordinating care across providers and community organizations preferred.

Responsibilities

• Conduct comprehensive member assessments to identify medical, behavioral, functional, and social needs.
• Develop, implement, and update individualized care plans.
• Meet with members in homes or community locations to coordinate care.
• Lead and collaborate with interdisciplinary teams to coordinate member care.
• Coordinate transitions of care and follow-up after hospitalizations to prevent readmissions.
• Connect members with housing, transportation, food, social services, and community programs.
• Educate members and families on Medicare/Medicaid benefits and preventive care.
• Monitor progress and adherence to care plans; document interactions and outcomes.
