Insight Global is seeking a Community Based Care Coordinator in Ohio to coordinate care for individuals eligible for Medicare and Medicaid. You will assess medical, behavioral, and social needs, develop individualized care plans, and connect members with resources while coordinating across providers and community organizations.

The role involves home visits, collaboration with interdisciplinary teams, education on benefits, and advocacy to support patient participation in care decisions.

Competenze

• Current clinical license in Ohio (RN or ASN).
• Experience in a healthcare-related profession is required.
• Valid driver's license with reliable vehicle.

Mansioni

• Assess medical, behavioral health, functional, and social needs of members.
• Develop and update individualized care plans.
• Coordinate care across physicians, providers, and community organizations.
• Educate members and families on Medicare/Medicaid benefits and preventive care.
