Job summary

Theoria Medical is seeking a full-time Advanced Practice Provider (NP/PA) in Wyoming, MI to serve local skilled nursing facilities. You’ll deliver patient-centered care with autonomy, supported by a collaborative team and modern tech, focusing on post-acute and SNF settings.

Qualifications

• Active U.S. license and board certification (NP or PA).
• NP/PA qualifications with relevant post-acute or SNF experience preferred.
• Ability to work full-time in Wyoming, MI.

Responsibilities

• Provide high-quality, patient-centered care in skilled nursing facilities.
• Conduct daily rounding and develop treatment plans.
• Prescribe medications and manage acute/emergent issues.
• Coordinate with facility staff via secure messaging or calls.
• Maintain on-site attendance for scheduled shifts and comply with standards.
