Theoria Medical is seeking an Advanced Practice Provider for full-time work in Battle Creek, MI to support local skilled nursing facilities. You will deliver patient-centered care, conduct rounds, and collaborate with a clinical team using intuitive technology.

Qualifications

• Active U.S. license and board certification in ANCC/AANPCB/PNCB/AACN for NPs or NCCPA for PAs.

Responsibilities

• Conduct daily rounding, prescribe medications and treatment plans.
• Communicate with facility staff via phone or secure messaging.
• Maintain reliable on-site attendance for scheduled shifts.
• Ensure compliance with clinical standards and facility policies.
