Job summary

Theoria Medical is seeking a full-time Advanced Practice Provider in Battle Creek, MI to deliver high-quality care in local skilled nursing facilities. You will conduct daily rounds, manage treatment plans, and collaborate with a multidisciplinary team to support vulnerable populations.

Qualifications

• Active U.S. license and board certification required (ANCC/AANPCB/PNCB/AACN for NPs or NCCPA for PAs).
• Advanced Practice Provider with specialties such as Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Geriatrics.

Responsibilities

• Provide patient-centered care to residents in skilled nursing facilities.
• Conduct daily rounds and manage treatment plans.
• Prescribe medications and address acute/emergent issues.
• Communicate with facility staff via secure messaging or phone.
• Maintain reliable attendance and comply with clinical standards.
