Job summary

Theoria Medical is seeking a qualified Advanced Practice Provider to join our Wyoming, MI post-acute and SNF program. Full-time on-site role with flexible scheduling and autonomy, supported by a collaborative clinical team and intuitive technology to deliver high-quality resident care.

You'll conduct daily rounding, prescribe treatments, and coordinate care with facility staff, while enjoying a competitive compensation package, benefits, and annual incentive opportunities.

Qualifications

• Active U.S. license and board-certified by ANCC/AANPCB/PNCB or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants).
• Specialties include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.

Responsibilities

• Provide high-quality, patient-centered care to residents in skilled nursing facilities.
• Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues.
• Communicate with facility nursing staff via phone or secure messaging as needed.
• Maintain reliable on-site attendance for scheduled shifts.
• Ensure compliance with clinical standards, facility policies, and regulatory requirements.
