The role emphasizes compassion, reliability, and teamwork. Compensation starts at $20.39 per hour with differential pay for specific schedules; requires 18+, HS diploma or GED, valid driver's license, and background checks;

Qualifications

• Must be at least 18 years old.
• High school diploma or GED.
• Valid driver's license to transport individuals in agency vehicles.
• Ability to complete background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and certifications.
• Commitment to a safe, healthy, professional work environment per Oak Hill policies.
• Commitment to continuity of care, including occasional shift extensions.

Responsibilities

• Assist with daily living activities including grooming, bathing, dressing, and hygiene per plans.
• Provide continence care as needed.
• Monitor health information and document vital signs and medications.
• Administer medications per training and nursing directives.
• Transport individuals to appointments and community activities.
• Assist with meals and nutritional needs.
• Support mobility and transfers using approved techniques and equipment.
• Document activities and medication administration in the EHR.
• Follow health care plans, safety protocols, and regulatory requirements.
• Communicate effectively with team, individuals served, and families.
