Qualifications

• High School Diploma required; CNA preferred.
• NADSP Certification must be obtained within the probationary period and maintained annually.
• Valid NY, PA, CT and NJ Driver’s License with 6 or fewer points.
• Strong communication, problem-solving, and organizational skills.
• Knowledge of developmental disabilities and regulatory standards (OPWDD/COA).
• Commitment to safety and confidentiality per HIPAA.

Responsibilities

• Assist with activities of daily living (ADLs) and mobility for residents.
• Execute behavioral support and monitoring plans and manage medical needs.
• Maintain accurate service records and ensure regulatory compliance.
• Facilitate community inclusion and participation in activities and life plan meetings.
• De-escalate crises and coordinate with clinical staff to ensure safety.
• Transport residents to appointments and activities as needed.
