Job summary

The Association for Individual Development in South Elgin, IL is seeking a DSP Case Manager to coordinate services for individuals with intellectual and developmental disabilities.

Qualifications

• High School diploma or GED required.
• Experience in case management for individuals with IDD preferred.
• Ability to document and report according to DHS, HCBS, and CARF standards.

Responsibilities

• Manage a caseload of up to 16 clients across I/DD services.
• Maintain client records in electronic system (Cx360).
• Coordinate health, behavioral, employment, and transportation services.
• Ensure rights and privacy under DHS, HCBS, CARF standards.
• Prepare referrals and submit for needed services.
