Job summary

OneWell Health Care is seeking a dedicated Direct Support Professional (DSP) or Caregiver to provide hands-on support in home and community settings around North Versailles, PA. The role emphasizes person-centered care, adherence to ISP/IPP plans, and promoting independence and inclusion for individuals with IDD and autism.

The DSP will work under supervision, travel to various homes and community sites, support daily living activities, and document care and progress while complying with HIPAA

Qualifications

• High school diploma or equivalent required or preferred.
• Experience in caregiving, home health, or direct support for IDD is preferred.
• Proficiency with documentation and HIPAA-compliant record-keeping is expected.

Responsibilities

• Provide hands-on, person-centered support in home/community settings.
• Assist with daily living activities including personal care, meals, mobility, and medication assistance as authorized.
• Implement behavior support strategies and monitor health, safety, and well-being.
• Maintain accurate documentation and attend trainings/meetings as required.
