Job summary

Healthfirst in Tennessee is hiring Care Coordinators to support members via care management teams or a call center. You will handle inbound calls, coordinate services, and document interactions in the PHI system.

Ideal candidates have CMA/CNA/RMA or related training, HS diploma, strong communication, and ability to multitask in a fast-paced environment. Preferred associates degree and experience in healthcare or call centers.

Qualifications

• CMA, CNA, or RMA OR relevant medical training or work experience.
• HS diploma/GED.
• Strong written and verbal communication skills.
• Ability to manage multiple priorities in a fast-paced environment.

Responsibilities

• Handle inbound calls from members regarding benefit questions/issues, care coordination, program inquiries, connecting to care manager, and screening assessments; document calls in PHI system.
• Manage assigned member caseload on care management teams, coordinating with interdisciplinary team to communicate member issues.
• Perform outreach calls to confirm services are in place.
• Support additional duties as assigned.
