Job summary

Qualifications

• CMA, CNA, or RMA certification or relevant medical training.

Responsibilities

• Care Coordinators support members in call center or care management teams.
• Document calls and care activities accurately in PHI system.
• Perform outreach calls to confirm services are in place.
• Collaborate with interdisciplinary care teams; perform other duties as assigned.
