Job summary

Healthfirst is seeking Care Coordinators to support members navigating the health care system. You will work with a Care Management team or a call center to coordinate services and document interactions in PHI systems.

Qualifications

• Certified Medical Assistant (CMA), CNA or RMA, or relevant medical training/work experience.
• HS diploma or GED.
• Excellent verbal and written communication; ability to handle multiple priorities.

Responsibilities

• For Call Center: handle incoming calls about benefits, care coordination, program inquiries, and connect to care managers; document calls in PHI system.
• For Care Management teams: manage a caseload and support timely communication within the interdisciplinary team to improve outcomes.
• Perform outreach calls to confirm services are in place for members.
• Other duties as assigned.
