Job Description

100% onsite. Requires working weekends and some holidays.

Primary Function

The Document Control Specialist is responsible for prepping, scanning, and indexing clinical paper documentation (medical records) for Jefferson Health Center City. This includes records for hospital and physician practices. The specialist performs quality reviews to ensure documentation includes clear patient identifiers, service dates, authentication, and complies with The Joint Commission, State, Federal Laws, and other regulatory requirements. They assign documentation to the correct patient, encounter, and document type in the EHR, providing timely, accurate, and accessible electronic patient health information. They collaborate with the Manager of HIM Document Management & Record Integrity to ensure all clinical documentation is accessible within the EHR within 24 hours of creation or other defined time frames.

Essential Functions
  • Prepares, scans, indexes, and performs quality analysis of paper documentation for hospital and Jefferson University Practices (JUP).
  • Ensures documents are aligned, maintained, and uniform in the EHR (Epic and Onbase).
  • Pick-up and deliver documents from medical practices and hospital units to HIM central location. Tracks and verifies receipt of discharged patient records.
  • Preps pages for scanning (removing staples, ensuring readability).
  • Verifies appropriate patient identification on every page of the document.
  • Scans medical record documentation following established procedures to ensure timely availability for patient care. Inspects the quality of each scanned document.
