Bachelor's degree or equivalent in social work or related human services field.
Two years of progressively responsible case management with adults, juveniles, and families.
Experience facilitating groups or collaborative meetings; able to handle 40+ caseloads.
Bi-lingual Spanish-English preferred.

Responsibilities

Coordinate diversion team services and restorative justice practices.
Collaborate with clinical staff to create individual participation plans.
Support participants to achieve program goals and progress tracking.
Coordinate with external treatment providers and manage releases of information.
Lead psychoeducation groups with licensed clinicians where appropriate.
Identify community resources and establish partnerships for ease of access.
