Job summary

Dayton Children's Hospital seeks an Integrated Care Resource Specialist to join the Family Resource Connection team. You will assist with post-discharge planning, linking families to community resources and coordinating care with the Integrated Care staff.

The role requires a Bachelor’s degree and 1–2 years in a related field, preferably in an acute care or outpatient setting. Work involves 10-hour shifts with one weekend day at the main campus.

Qualifications

• Bachelor’s degree required
• Bachelor’s degree related field (social work, psychology, or education) preferred
• 1-2 years or related field experience required
• Experience in acute care hospital or outpatient clinical services preferred
• Experience in case management and/or denials prevention programs preferred

Responsibilities

• Coordinate post-discharge placement needs with patients and families
• Assist in identifying and problem solving community resources to support discharge
• Receive consults, screen needed services, and share handoff information with Integrated Care staff
• Organize and update information on existing and new community resources for the department
• Support the Child Passenger Safety Program by becoming a certified technician
