Job summary

Dayton Children's Hospital is seeking a Medical Social Worker for the Social Work department on the Main Campus Neurology unit. The role involves coordinating care transitions with RN Case Management and supporting discharge planning.

Requires an MSW from an accredited program and Ohio LSW licensure, with 1–2 years’ healthcare experience preferred. Work hours are 8:00 AM–4:30 PM, Monday–Friday, in a full-time schedule.

Qualifications

• Masters Degree in Social Work from an accredited institution.
• 1-2 years working within Healthcare or Hospital preferred.
• Licensed Social Worker (LSW) in Ohio required.

Responsibilities

• Coordinate care transitions with the RN Case Manager and Integrated Care staff.
• Assist patients and families in understanding and coping with illness and disability.
• Collaborate with hospital and community agency personnel for care planning.
• Provide education/consultation to health care professionals and the community on psychosocial factors.
• Support onboarding, training, and mentoring for newer staff and students.
