Responsibilities

• Monitors admission and continued stay of all patients.
• Coordinate discharge planning with families and multidisciplinary team.
• Collaborate with physicians to develop discharge goals and plans.
• Coordinate medications, equipment, and home care services as needed.
• Assist with follow-up appointments and therapeutic scheduling.
