Qualifications

Responsibilities

• Assist with scheduling of inpatient and outpatient services, tests, procedures, and follow-up appointments.
• Communicate with patients and families regarding admissions and appointments and connects them with appropriate resources.
• Collaborate with multidisciplinary teams to ensure cohesive discharge planning and effective communication throughout the discharge process.
• Obtains required information from patients, caregivers, healthcare providers, and other external teams.
• Assist with completing required documentation such as FMLA forms and obtaining payer authorizations for medications, equipment, and services related to post-discharge care.
• Monitors related metrics and assists with data gathering to support process and patient care improvements.
• Ensures compliance with regulatory and organizational privacy and safety standards.
