Job summary

Family & Children’s Services in Oklahoma City is seeking a coordinator to manage discharge transitions from the Urgent Recovery Center (C3) to Hope outpatient services or to another appropriate level of care.

The role also handles capacity management of chairs and beds at the Urgent Recovery Center and supports collaboration with other team members to ensure continuity of service during client transfers.

Qualifications

• Bachelor’s degree in the social sciences preferred; psychology or related field acceptable.
• Ability to establish supportive relationships with individuals with mental illness or substance issues.
• Valid driver's license with satisfactory driving record; use of personal vehicle for travel.

Responsibilities

• Ensure seamless discharge transitions from Urgent Recovery Center (C3) to Hope outpatient services or other appropriate care.
• Manage capacity of chairs and beds at the Urgent Recovery Center and monitor community hospital bed availability.
• Collaborate with team members to ensure continuity of service during client transfers to other MHC locations or agencies.
