Job summary

Dayton Children's is seeking an Integrated Care Resource Specialist to support discharge planning and connect families with post-discharge resources within a multidisciplinary team.

You will screen services, gather handoff information, coordinate community resources, and liaise with departments and external partners to ensure timely care while addressing complex family needs. Bachelor's degree required; 1–2 years in acute care or case management preferred.

Qualifications

• Bachelor’s degree is required.
• Experience in acute care hospital or outpatient clinical services preferred.

Responsibilities

• Assist in facilitating and linking patients and families with post-discharge placement needs.
• Screen needed services and provide handoff information to Integrated Care staff.
• Organize information about community resources and share updates with the department.
