Job summary

Dayton Children's Hospital is seeking an Integrated Care Resource Specialist for the Family Resource Connection at the Main Campus. This role supports post-discharge planning, connects families with community resources, and collaborates with care teams to ensure timely services.

A Bachelor’s degree is required, with 1–2 years of related field experience preferred, especially in acute care or case management.

Qualifications

• Bachelor’s degree required.
• Bachelor’s degree related field (social work, psychology, or education) preferred.
• 1–2 years of related field experience required.
• Experience in acute care hospital or outpatient clinical services preferred.
• Experience in case management and/or denials prevention programs preferred.

Responsibilities

• Coordinate post-discharge placement and connect families with community resources.
• Receive consults and screen needed services, ensuring timely care transitions.
• Organize and share updates on community resources and new services with the department.
• Serve as a liaison with external partner agencies and hospital departments.
• Support the Integrated Care team in prioritizing tasks and managing complex family dynamics.
