Job summary

The Providence at Home with Compassus is seeking a Clinical Care Partner to coordinate safe, efficient transitions of care for hospitalized patients. This in-person role focuses on discharge planning, post-acute home-based care decisions, and collaboration with physicians, case management, patients, and families.

You will evaluate patients for home health, hospice, and DME needs, arrange post-acute services, educate patients and families, and support value-based care initiatives through accurate

Qualifications

• Required: Associate’s degree in Nursing, Health Sciences, or related field; equivalent healthcare experience accepted.
• Preferred: Bachelor’s degree in nursing, Health Sciences, or related field.
• Preferred: 2–3 years in care coordination, discharge planning, or healthcare services; experience with EMR systems and referral platforms.

Responsibilities

• Coordinate safe, timely discharge planning and transitions of care.
• Evaluate patients for home-based and post-acute care services.
• Collaborate with physicians, case management, and nursing teams.
• Educate patients and families on post-acute options.
• Maintain documentation in the EMR and manage referrals.
• Participate in interdisciplinary rounds and quality improvement initiatives.
