UPMC Mercy is seeking a Full Time Discharge Planning Coordinator to support the Clinical Care Coordination and Discharge Planning team. This role focuses on clerical support and coordination to ensure safe, timely discharges.

You will work Monday through Friday, 8am to 4:30pm, performing chart reviews, coordinating materials, and serving as a liaison between patients and the discharge planning staff. This is a non-clinical role with HIPAA compliance and collaboration with the care team to remove

Responsibilities

• Performs clerical tasks to support discharge planning staff (copying, faxing, scheduling).
• Provides forms, resources and information required for discharge plans to patients/caregivers as directed.
• Uses electronic medical records to complete chart reviews, collect data, document progress, and track communication.
