• RN or social worker with discharge planning experience.

Responsibilities

• Identify clinical, psychosocial, financial, and other needs to guide discharge planning.
• Complete detailed assessments to determine capacity for self-care and post-hospital needs.
• Coordinate safe and efficient discharges with the care team and post-hospital providers.
• Communicate clearly with patients, families, and the care team to develop individualized discharge plans.
• Serve as a liaison between hospitals and post‑hospital care facilities and physicians.
• Ensure compliance with regulatory requirements and organizational values.
• Participate in process improvement and staff mentorship.
