The role supports a patient-centered discharge process with a focus on timely utilization of resources and collaboration with physicians or APPs.

The position emphasizes discharge planning, care coordination, and acting as the central point of contact through the patient’s care journey, working with RNs and social workers

Qualifications

• RN or social worker with care coordination or discharge planning exposure.
• Experience in discharge planning or case management is preferred.

Responsibilities

• Identify patient needs (clinical, psychosocial, financial) to guide discharge planning.
• Assess capacity for self-care and determine need for post-hospital services.
• Coordinate with care team to arrange safe, timely discharge.
• Communicate discharge plans clearly with patient, family, and care team.
• Serve as liaison between hospital and post-acute care providers.
• Ensure discharge plans align with patient goals and available resources.
