Job summary

UPMC Mercy is seeking a Full Time Discharge Plan Manager to support the Emergency Department. This role involves coordinating post-discharge needs within a multidisciplinary team, with RNs and social workers sharing discharge planning responsibilities alongside physicians or APPs.

Qualifications

• RN or licensed social worker needed or interested in care management, case management or care coordination.
• Experience in discharge planning or care coordination preferred.
• Willingness to work varied 12-hour shifts with 24/7 coverage, including rotating weekends/holidays.

Responsibilities

• Identify clinical, psychosocial, financial, and other needs to guide the discharge plan.
• Assess patient capacity for self-care and plan post-hospital services as needed.
• Coordinate with care teams to ensure timely discharge and minimize delays.
• Communicate effectively with patients, families, and interdisciplinary teams throughout the discharge process.
• Act as liaison between hospital and post-hospital facilities and care providers.
• Support department operations, mentor staff, and lead process improvement initiatives.
