UPMC Mercy is hiring a Full Time Discharge Plan Manager to support the Emergency Department. This position will work varied 12-hour shifts with 24/7 coverage and rotating weekends/holidays.

Qualifications

• RN or social worker with interest in care coordination and discharge planning.
• Experience in discharge planning/care coordination preferred.
• CCM/ACM or related certification preferred.

Responsibilities

• Identify patient needs across clinical, psychosocial, financial and social domains to plan discharge.
• Complete detailed assessments to determine capacity for self-care and post-hospital needs.
• Coordinate safe and efficient discharges with the care team and post-hospital resources.
• Communicate with patients, families and care teams to develop individualized discharge plans.
• Serve as liaison between hospital and post-acute care facilities.
• Align discharge plans with organizational mission and standards.
• Participate in staff mentoring, process improvement and department operations.
