Responsibilities

• Performs clerical tasks and other non-clinical duties to support discharge planning.
• Provides forms, resources, and information to patients/caregivers as directed.
• Uses electronic medical records to review charts, collect data, document progress, and track communication.
• Serves as a contact between patients and discharge planning staff.
• Gathers and arranges materials needed to enable patient discharge.
• Order supplies/equipment for patient-specific needs identified by the clinical team.
• Complies with system-wide competencies and HIPAA requirements.
• Reports information clearly and in a timely manner.
• Alerts the team to concerns related to the plan of care and discharge barriers.
• Assists the team with questions and troubleshooting.
