Job summary

USA Health is seeking a qualified RN to join our Discharge Planning team on the Gulf Coast. The discharge planner will coordinate assessments and collaborate with the healthcare team to build effective post-acute plans for patients.

Responsibilities include clear communication with physicians, patients and families, responding to overhead pages, and contributing to performance improvement efforts. A valid Alabama RN license and 2 years of nursing experience are required.

Qualifications

• Graduation from an accredited school of nursing meeting the minimum standards set by the State of Alabama and 2 years professional nursing experience required.
• RN - Registered Nurse - State Licensure and/or Compact State Licensure in accordance with the Nurse Licensure Compact (NLC) for Alabama required.
• Comparable combination of education and experience may substitute for the above requirements.

Responsibilities

• Communicates and uses appropriate customer relation skills with physicians, patients, families and healthcare team in person, via email, via telephone, or other communication application.
• Responds to overhead pages.
• Participates in performance improvement activities as identified and assigned.
• Completes all mandatory department, educational and hospital requirements.
• Adheres to hospital policies including confidentiality.
