Job summary

Northwest Texas Healthcare System is seeking a Discharge Planner to coordinate social services and discharge planning for patients and families, contributing to continuity of care. You will work with the healthcare team to evaluate needs and develop discharge plans.

Responsibilities include conducting pre-discharge interviews, assessing living situations and financial status, and creating viable intervention plans to ensure safe, timely discharges.

Qualifications

• Bachelor’s degree in social work or related field; two years’ experience preferred.

Responsibilities

• Completes pre-discharge evaluations from patient/family interviews and healthcare team collaborations.
• Develops a plan of intervention for discharge considering illness/injury, living situation, support systems, and financial status.
