Job summary

Advocate Health is seeking an experienced RN in a leadership role for unit coordination and discharge planning. The incumbent will guide staff, ensure safe patient care plans, and support throughput and patient flow while maintaining high clinical standards.

The role requires a current Georgia RN license, a BSN preferred, and relevant bedside experience including charge nurse duties. Hospital-based position with a focus on safe discharge planning and staff development.

Qualifications

• Current RN license with the Georgia State Board of Nursing.
• Graduate of an accredited nursing program; BSN preferred.
• Minimum 6 months experience as a Clinical Nurse; 2 years preferred; 6 months as Charge Nurse.

Responsibilities

• Provides leadership to the health care team with emphasis on coordination of patient care and throughput of patients.
• Participates in staff performance evaluation and alignment of educational activities with goals.
• Assures orientation and competency checks for new staff and organizes staff development.
• Facilitates discharge planning and ensures post-discharge actions are coordinated with post-acute care.
