You will assess pre-discharge needs and collaborate with the healthcare team to plan intervention based on patient circumstances and financial status.

• Bachelor’s degree in social work or in related field is required or preferred.
• Two years experience in related field is preferred.

Responsabilidades

• Completes pre-discharge evaluations from patient/family interviews and healthcare team collaborations.
• Based on evaluation, develops a viable plan of intervention for current needs and potential discharge needs considering illness/injury, living situation, support systems, and financial status.

Educación

Bachelor’s degree in social work or related field
